เฮพารินโคแฟกเตอร์ II (อังกฤษ: Heparin cofactor II หรือ HCII) เป็นโปรตีนที่พบในมนุษย์ ถูกถอดรหัสได้จากยีน SERPIND1 บน ถูกจัดอยู่เซอร์ปิน เคลดดี ลำดับที่ 1 มีฤทธิ์เป็น (serine proteinase inhibitor) ทั้งนี้ HCII เป็นสารตัวกลางที่มีความจำเป็นต่อกระบวนการการจับลิ่มของเลือด โดยจะออกฤทธิ์ยับยั้งแฟกเตอร์ IIa และเป็นโคแฟกเตอร์ต่อเฮพารินและ นอกจากนี้ยังสามารถต้านได้เล็กน้อย โดย HCII จะสามารถออกฤทธิ์ยับยั้งการทำงานของทรอมบินได้อย่างรวดเร็วเมื่อมีการแตกออกของเฮพารินจากแมสต์เซลล์หรือมีการฉีกขาดของที่ผนังหลอดเลือด

ยีน SERPIND1 ที่ควบคุมการถอดรหัส HCII นั้น มีส่วนประกอบที่สำคัญเป็น 4 เอ็กซอน และ 5 อินตรอน การวิเคราะห์หลักฐานกายวิภาคเปรียบเทียบพบว่า HCII มีการพัฒนาโครงสร้างมาจากจุดกำเนิดเดียวกันกับแอนติทรอมบินและโปรตีนสมาชิกอื่นในกลุ่มแอลฟา-1 แอนติทริปซิน แต่ทำหน้าที่ต่างกัน (homology) โดยการกลายพันธุ์ของยีนนี้มีความสัมพันธ์กับการเกิดภาวะพร่องเฮพารินโคแฟกเตอร์ II (heparin cofactor II deficiency) ซึ่งจะทำให้มีการสร้างที่มากเกินและเกิดภาวะเลือดแข็งตัวเร็วกว่าปกติได้ (Hypercoagulable state)
