ลิลี อิลเซ เอลวีนส์ (เดนมาร์ก: Lili Ilse Elvenes; 28 ธันวาคม ค.ศ. 1882 – 13 กันยายน ค.ศ. 1931) หรือรู้จักกันในนาม ลิลี เอลบี (Lili Elbe) เป็นจิตรกรหญิงข้ามเพศชาวเดนมาร์กและเป็นหนึ่งในผู้เข้ารับในยุคแรก ๆ

เธอเป็นจิตรกรภายใต้ชื่อเกิดของเธอ หลังผ่าตัดแปลงเพศในปี 1930 เธอเลิกเป็นจิตรกรและเปลี่ยนชื่อทางกฎหมายเป็น ลิลี อิลเซ เอลวีนส์ และใช้นามสกุลว่า เอลบี ในเวลาต่อมา เธอถือเป็นผู้เข้ารับคนแรกที่พยายามจะตั้งครรภ์ แต่ภายหลังเสียชีวิตเนื่องจากภาวะแทรกซ้อนดังกล่าวตามมา
